Forever Dental Care accepts all traditional insurance plans in which you are able to choose your dentist. If you have any questions regarding your dental insurance please feel free to call or email our office with your questions.

We are contracted with specific insurance carriers to provide services at a discount. The insurance will pay a percentage of the services we provide for you based upon previously agreed amounts with the dental carrier.

We do not accept Molina or Priority Health Medicaid or participate with any HMO (Health Maintenance Organization) or DMO (Dental Maintenance Organization). Patients with those plans must be seen by an HMO or DMO provider in that particular network.
